Telebelly Health (“Telebelly”) understands the importance of your privacy and protecting your personal information.  The purpose of this Privacy Policy (the “Policy”) is to describe how Telebelly collects, uses, and shares information about you through our website, social media, email exchanges, mobile apps, and other online services on which this Policy is posted (the “Service”), as well as how you can access, change, and remove your information. Please read this Policy carefully to understand what we do. If you do not understand or agree to any aspects of our Policy, please contact us before continuing to use our Service.

This Policy is written in the English language. We do not guarantee the accuracy of any translated versions of this Policy. To the extent that any translated versions of this Policy conflict with the English language version, the English language version of this Policy shall control.

Information that does not identify you is not your “personal information” for purposes of this Policy.

CHILDREN’S USE OF THE SERVICE

If you are under the age of 18, please do not register for the Service or provide any personal information about yourself to us. COLLECTION OF YOUR PERSONAL INFORMATION

Information you provide to us. When you schedule an appointment with a provider, we may ask you to provide the following information: (1) your contact information, including your first and last name, phone number, address, home state, and email address; (2) authentication information to keep your account safe, including the user name and password that you use to register your account; (3) financial information, including your debit or credit card number, its expiration date, and its security code, for payment processing purposes; (4) personal characteristics, including date of birth; (5) information relevant to your diagnosis and treatment, including general medical history and electronic health records from other providers; (5) comments, reviews, and suggestions about our Service; (6) personal preferences including product preferences, online preferences, and interests; (7) online behavior information including online activity, preferences, and time spent viewing features; and (8) IP address, mobile network information, or device information.

Information from third-party sources. We may also collect information about you from companies that collect or analyze information about consumers or users, such as demographic data, web traffic data, or data provided from sources that you have authorized to share such data with us, either through our Service or via third-party applications or websites.

Information we collect automatically.  We may collect data regarding your use of the Service through cookies, web beacons, and other automatically collected information. This data may include your IP address; date and time you access the Service and the pages and content you access during your visit; websites that you link to or from; emails from us that you open and the links you click on within those emails; and device information.

  • To provide the medical providers in our associated physician practice, Telebelly PLLC, with information relevant to your treatment.
  • To operate our business.
  • For payment processing.
  • To provide, improve, maintain, or optimize our Service.
  • To provide customer support and respond to your inquiries.
  • To comply with law and satisfy our compliance obligations.
  • To detect and prevent fraud and other prohibited or illegal activity.
  • To protect the rights and interests of you, ourselves, and others.
  • For benchmarking and analysis.
  • To deliver content and marketing communications that we think may interest you. You may opt out of receiving marketing emails from us by following the unsubscribe link in each email, or by emailing privacy@telebelly.com. You may continue to receive non-marketing emails from us after you opt-out.
HOW WE SHARE YOUR PERSONAL INFORMATION WITH THIRD PARTIES

We may share your personal information with third parties for certain purposes, including:

  • With medical providers who provide health care services, including those scheduled through our Service, to provide treatment to you.
  • With vendors and service providers with which we partner to complete transactions or process payments you have authorized. Currently, this includes Stripe, which is a payments processing vendor. These service providers store your personal information subject to the privacy policies and practices of the service provider and such practices are not subject to the terms of this privacy notice.
  • With our professional advisors to provide legal, compliance, accounting, banking, or consulting services.
  • In order to comply with our legal obligations or to protect our interests, property or legal rights, or those of our customers or third parties, such as to enforce our terms and conditions, or to exercise or defend legal claims.
  • With law enforcement, officials, or other third parties when we are compelled to do so by a subpoena, court order, or similar law enforcement request, or when we believe in good faith that the disclosure of personal information is necessary to prevent physical harm or financial loss, to report suspected illegal activity, or to investigate violations of this Privacy Notice or other applicable terms.
  • With companies or other entities in connection with, or during the negotiation of, any merger, sale of company stock or assets, financing, acquisition, divestiture or dissolution of all or a portion of our business.
  • With your direction or consent.

Other parties may collect personal information about your online activities over time and across third-party websites. We do not respond to “do not track” signals or other mechanisms that provide consumers the ability to exercise choice regarding the collection of personal information about a consumer’s online activities over time and across third-party websites.

Telebelly uses Google Analytics. Google Analytics is a web analytics service provided by Google, Inc. to collect certain information relating to your use of the Service. Google Analytics uses cookies to help us analyze user activity.

Telebelly does not trade, rent, or sell your personal information to third parties.

HOW WE SECURE YOUR PERSONAL INFORMATION

We are committed to maintaining measures to protect the security of your information. Of course, despite these measures, no network or system is ever entirely secure and we cannot guarantee the security of networks and systems that we operate or that are operated on our behalf. Telebelly cannot promise or guarantee that hackers, cybercriminals, or other unauthorized third parties will not be able to defeat our security, and improperly collect, access, steal, or modify your personal information. Please protect the security of your login information, including your username and password.  Also, please note that emails and other communications you send to us through are not encrypted, and we strongly advise you not to communicate any confidential information through these means.  

SHOULD YOU USE SOCIAL MEDIA SIGN-ONS?

To the extent that you choose to use a social media account application (such as Google, Facebook, or Apple) to create your online account with Telebelly or otherwise sign into our Service, you understand that if another person has access to your social media account, they will also have access to your account on our Service. That means that other person could access any personal information contained on the Site. It’s your decision about whether to give another person access to your social media account and whether to use that account to sign on to the Site. Also, if you use a social media account application to sign into the Site, you understand that the social media account application may send information from your social media account to Telebelly.

HOW CAN YOU ACCESS YOUR PERSONAL INFORMATION?  

You can access and update information relating to your online account by signing into your account and going to the Account section. If you have questions about the personal information we hold about you or need to update the personal information in your account, you can contact us.

UPDATES TO OUR PRIVACY POLICY

We may update this Policy from time to time. If we make changes, we will revise the Effective Date at the top of this Policy. Changes to this Policy will be effective once they are posted unless otherwise indicated.
